Parental Authorization is ALWAYS required when minors are participating in:

• overnight events
• specialized activities outside of normal weekly programming
• out-of-town events

Additionally, authorization is required for Children’s ministry when children are being dropped off and parents are not staying on site during the event. This applies to events held at the church building, as well as offsite events.

For a new event registration, you can use the Form Template titled “Event with Parental Authorization” or you can follow the instructions below to build your own form.

If you choose to build your own form, the verbiage and questions below must be included on the final page of the CCB registration form for any event that meets the criteria listed above.

These questions should be included in the Parental Authorization section of the form and not repeated in any other areas of the form. If your ministry would like to ask additional questions (ex: parent/guardian info for 2 people), you can add more fields. Do not omit any of the questions listed below.
