What’s the purpose of 360 Reviews?
Employee evaluations will be conducted after the end of the first year of employment and annually thereafter. A standard evaluation format will be used for employee evaluations. The purpose of evaluations is to provide encouragement and constructive feedback on staff performance. It is also an opportunity for the employee to give feedback.
What’s the process for completing 360 Reviews?
- HR will email you stating that the process has started with deadlines.
- Fill out a form requesting the coworkers that you want to review you.
- Fill out a self Review Form.
- Fill out any Co-workers review forms for others.
- Your supervisor should get in contact you about setting up a time to meet to go over the Employee Evaluation Report.
- Your supervisor and you will set action goals for the following year.
What’s included in 360 Reviews?
- One Self Review
- Three Peer (Co-worker) Reviews
- Employee Evaluation Report (EER): a summary of the reviews along with your superior review
- A formal meeting with you and your supervisor

When will they occur?
The process will start in November and will be completed by the end of the calendar year.
How many times should you meet with your supervisor?
At least once a month if not more depending on your conversations. If your supervisor isn’t consistently checking in with you (formally or informally), please notify Human Resources.
